Continuing Care
You and your family will find the counseling of the case manager helpful if you continue to need care after you leave the Hospital. The case manager is familiar with the availability of rooms in nursing homes and other helpful information, such as where to get medical equipment. Your physician may also refer you to a home health care agency which provides skilled nursing care and other services. Vital Care, Inc. is the home health service associated with Cheboygan Memorial Hospital.
Patient education often continues after discharge. Support groups and teaching classes are available for patients with, for example, diabetes, mastectomy, lung and heart disease. Your physician, nurse or the Case Manager will give you more information on care which is of particular interest to you.
Community and Home Health Services
Various agencies in the area provide home health services, which may include skilled and psychiatric nursing, IV therapy, medical social services, physical therapy, occupational therapy, speech therapy, home health aide assistance, maternal and infant support, Alzheimer's support, rehabilitation therapy and respite care services.
For your convenience, home health agencies will invoice Medicare, Medicaid, Blue Cross and other third-party payers directly. The Case Manager will give you more information and assist you in arranging for your continuing care. Home health agencies which service the area include: VitalCare, District Health Department #4, LMAS District Health Department, Northwest Community Mental Health Agency, Northwoods Home Nursing, Munson Home Health, Heartland Home Health Care, Miji, Chippewa County Health Department, and 24-Hour Live-in services.
Extended Care
Larson Hall is a 50-bed skilled nursing, long-term care unit attached to Cheboygan Memorial Hospital, (231) 627-1242.
Tendercare is a 112-bed skilled/non-skilled, long-term care facility located in the City of Cheboygan.
You may wish to obtain skilled nursing care in a facility located in a neighboring community. The Case Manager has information about these facilities and can help make arrangements for you. Consult your physician for more information about extended care.
Adult Foster Care Homes
If you are unableĀ to live alone, but do not need basic or skilled nursing home care, you may wish to go directly to an Adult Foster Care home. The Case Manager can assist you with locating a home in the community.
Hospice
Hospice is a special program of care, usually provided in the home, for the terminally ill patient and his/her family. Hospice care is coordinated with the patient's physician and includes medical and nursing care, symptom and pain management, home health and homemaker services, transportation, personal care and supportive visiting, spiritual guidance and emotional and bereavement support. Hospice care can be provided by Hospice of the Straits, Northwest Michigan Community Health Agency, LMAS District Health Department, Northwoods Home Nursing and Hospice, and Chippewa County Health Department.
Hospice of the Straits also operates a Hospice House in Cheboygan for patients who have no primary care giver at home.
